Even if we do not carry your insurance, we offer courtesy discounts. If you do not see your insurance provider, we are happy to submit all of the paperwork to your insurance provider for your direct reimbursement.

Now that you’ve learned about us, it’s time for us to get to know you. If you wish to schedule an appointment at Price Optical, please print and complete the necessary paperwork. It includes our Patient Information Form. It also includes a Medical Information Questionnaire, an explanation of our Optomap Retinal Scan Technology, and a copy of our Privacy Practices.
